The additional power (Molex or 2.1 barrel) is just for feeding the 5V in the USB not for powering the card itself.
If you want to use external power to power the card instead of the pci-bus then you would need to buy a riser card with external power. The external power of the riser will feed the pci-card but not the 5Von the USB.

My first assumption was that the internal molex is to power the card and the external 2.1 jack is for powering the USB's! Bad, bad me! :(

the usb card itself is more than likely powered with 3.3 volts, difficult to find an off the shelf linear supply for that
and then still need a 5v external for the port

Well, no - I should have been more careful in my statement & changed it to say that some (not all) USB devices actual regulate that incoming 5V down to 3.3V (but this would need to be checked for the device in question).
