We use Etymotic ER4Ps (I bought the S adapter but never used it), very happy with them. A few friends who have tried them, when looking at in-ear headphone, nearly all ending up buying a set.
Never buy these at MSRP, you can always get them cheaper on Audiogon and Amazon
These tend to have a pickup effect though, the current models less so then previous ones but it is still there to some extent. If you jog etc. with them you might find it annoying. The noise canceling while very useful, especially on planes etc. can also make these unsuitable for some outdoor use.
